Address lookup on any address entry page

We have used on our internal web based systems for many years and it is very easy to implement and works well. You can search on any field. From memory you (a) create an account with Loqate, (b) get a key, (c) You then add credit, (d) you then add a few lines of code into each page that contains an address form. I have attached an example of what it looks like.
